Position: Case Manager /HIV Substance Abuse

HIV Substance Abuse Case Manager is responsible for conducting a brief substance abuse assessment for patients with HIV and identifying those with a significant substance abuse history. Additional duties include referrals and linkages to substance abuse providers and monitoring patient progress in treatment.


MAJOR DUTIES/ESSENTIAL FUNCTIONS

The major duties will include, but are not limited to:

· Conducts an extensive substance abuse assessment for HIV patients with Substance abuse issues to determine the level of service needed.

· Develops active substance abuse treatment plans within 30 days of assessment and monitors plans every 90 days.

· Liaises with all Unity HIV providers including physicians, nurses, and case managers to identify patients with substance abuse issues and services them.

Case Manager-Required Education/Required Training: High School Diploma or equivalent with minimum of 5 years work related experience with a difficult population, preferred. Certified Addictions Counselor certificate with registration in DC preferred. Bachelor's Degree in Human Services or related field preferred.

Case Manager III -Master's degree in Human Services or related field, LICSW or LPC required

Experience working with underserved or homeless populations preferred. * For correctional position-correctional experience- preferred but not required.
